We accept referral from Statutory Team where Shared Lives has been identified to be able to meet the Individual`s need following an assessment.
We would require up to date care and support assessment and Care plan.
If you would like to receive more information on service cost and cost to the Individual please contact Shared Lives on sharedlives@valeofglamorgan.gov.uk.
We accept referrals from out of county commissioning Teams.
